explaining clindamycin

What is clindamycin (Cleocin) and what is it used for?

Topical clindamycin, also known as Cleocin, is a prescription antibiotic medication applied directly to the skin.

It is primarily used to treat acne, especially inflammatory acne with red bumps, pustules, and tenderness. Topical clindamycin works by reducing acne-causing bacteria on the skin and helping to calm inflammation.

In some cases, topical clindamycin may also be used to treat folliculitis and mild, localized bacterial skin infections caused by susceptible bacteria. Because it acts locally with minimal absorption into the bloodstream, it is commonly chosen for skin conditions that do not require oral antibiotics.

Topical clindamycin is available in several forms, including gels, lotions, creams, foams, and solutions, and is typically used once or twice daily as prescribed by a healthcare professional.

Why is clindamycin often combined with other acne treatments?

Clindamycin is an antibiotic that helps reduce acne-causing bacteria and inflammation. To limit the risk of antibiotic resistance, dermatologists typically recommend using clindamycin in combination with other non-antibiotic acne treatments, rather than on its own for long periods.

Common companion ingredients include benzoyl peroxide, which helps prevent resistant bacteria, and topical retinoids, which keep pores clear and improve long-term acne control. Using combination therapy allows clindamycin to remain effective while targeting multiple causes of acne.

How to apply clindamycin?

According to Dr. Trisha Khanna, a board-certified dermatologist, if you have been prescribed with clindamycin, this is how you should use it.

1. Cleanse gently

Gently cleanse the skin and pat dry before application

2. Use a very small amount

Apply a thin layer to the affected area, not just individual pimples

3. Use as prescribed

Use exactly as prescribed by your dermatologist (commonly once daily)

4. Do Not Overuse

Avoid applying more often than directed, as this does not improve results and may increase irritation

6. Avoid Sensitive Areas

Avoid the eyes, lips, and inside of the nose

7. Moisturize After Use

Follow with a non-comedogenic moisturizer to help reduce dryness or irritation

8. Use sunscreen daily

Use sunscreen (SPF 30 or higher) each morning, as treated skin may be more sensitive to sun exposure

9. Clean Hands After Application

Be sure to wash your hands after applying the product to avoid accidental contact with sensitive areas.

Clindamycin and other medications

Ingredients that can be used alongside clindamycin

Included Benzoyl peroxide – especially important with clindamycin to help reduce acne-causing bacteria and lower the risk of antibiotic resistance (often used at a different time of day or in a fixed-dose combination)

Included Topical retinoids (tretinoin, adapalene, tazarotene) – help prevent clogged pores and improve long-term acne control; typically used at night while clindamycin is used in the morning

Included Niacinamide – helps calm inflammation and support the skin barrier

Included Azelaic acid – addresses acne, redness, and post-acne marks and is generally well tolerated

Included Gentle, non-comedogenic moisturizers (ceramides, glycerin, hyaluronic acid) – help maintain the skin barrier during treatment

Included Broad-spectrum sunscreen (SPF 30+) – protects the skin and helps prevent post-inflammatory hyperpigmentation

Ingredients to avoid or use cautiously with clindamycin

Warning Strong exfoliating acids (high-strength AHAs or frequent BHA use) – may increase dryness and irritation when combined

Warning Physical exfoliants (scrubs, brushes) – can worsen redness and compromise the skin barrier

Warning Alcohol-heavy or astringent products – may increase dryness and irritation

Warning Sulfur (especially in sensitive skin) – may be overly drying when combined with clindamycin

What a technology! I was very impressed by the speed of communication with a doctor using the Miiskin app. Losing my hair has been my biggest concern, but I didn’t have the courage to reveal my bald spot to a doctor in person. Miiskin solved all my issues instantly. I just took a photo in my own room, uploaded it, and wrote about my concerns. That’s it. The best part is that without insurance, the consultation fee was much lower than any health insurance. I submitted my photo at midnight and received a prescription the very next morning. The doctor at Miiskin was very professional and empathetic, which really helped ease my emotional distress. Although I’m just at the beginning stage of my treatment and haven’t seen significant results yet, I feel reassured and supported. I will definitely upload my progress in 3 to 4 months to show my appreciation for the prescription from Miiskin.

Quick response from the dermatologist — I wanted a fast turnaround for a skin concern that would be more immediate than going through health insurance, making an appointment, traveling to the doctor.

Got more questions about getting a clindamycin prescription online?

How much does clindamycin cost?

In the U.S., topical clindamycin prices vary by formulation. Generic clindamycin 1% typically costs $25–$80 without insurance and may be lower with discounts. Brand-name products are more expensive, for example, Clindagel® can cost $200+, and clindamycin combination products such as Benzaclin®, Duac®, or Acanya® often range from $200–$300+ without insurance.

Is clindamycin oral and topical?

Yes. Clindamycin is available in both oral and topical forms.
Clindamycin can be prescribed orally, usually as capsules or liquid, to treat moderate to severe bacterial infections throughout the body, such as skin, dental, bone, or respiratory infections.
It is also commonly prescribed topically as a cream, gel, lotion, foam, or solution. Topical clindamycin is most often used to treat acne and other inflammatory skin conditions, as it works locally on the skin with minimal absorption into the bloodstream.
The choice between oral and topical clindamycin depends on the type and severity of the condition being treated, as determined by a healthcare professional

Is oral clindamycin also used in dermatology?

Yes. Oral clindamycin is also used in dermatology, though it is prescribed less commonly than topical clindamycin.
In dermatology, oral clindamycin may be used to treat moderate to severe skin and soft tissue infections, such as cellulitis, abscesses, or infected wounds caused by susceptible bacteria. It can also be used in certain inflammatory skin conditions or severe acne cases, often in combination with other treatments, when topical therapies alone are not sufficient.
Because oral clindamycin affects the whole body and carries a higher risk of side effects, particularly gastrointestinal side effects, dermatologists typically reserve it for situations where the benefits clearly outweigh the risks.

Can you drink alcohol while using topical clindamycin?

Dr. Khanna explains that alcohol consumption does not interact with topical clindamycin because very little of the medication is absorbed into the bloodstream. However, alcohol may worsen skin inflammation or acne in some individuals, so moderation is recommended.

How long does topical clindamycin stay in your system?

According to Dr. Khanna, topical clindamycin has minimal systemic absorption. Small amounts may enter the bloodstream, but they are usually eliminated within 24 hours. The medication continues to work on skin bacteria as long as it is applied regularly.

What does topical clindamycin treat?

Dr. Khanna notes that topical clindamycin is primarily used to treat acne, particularly inflammatory acne with red, tender bumps and pustules. It may also be prescribed for certain localized bacterial skin infections when caused by susceptible bacteria.

How long does topical clindamycin take to work?

Dr. Khanna explains that topical clindamycin begins reducing acne-causing bacteria soon after starting treatment. Visible improvement in acne typically appears within 4 to 8 weeks, with continued improvement over time when used consistently.

How is topical clindamycin prescribed?

Dr. Khanna states that topical clindamycin is prescribed as a gel, lotion, cream, foam, or solution. It is usually applied once or twice daily to clean, dry skin, depending on the formulation and the patient’s skin sensitivity.

Is topical clindamycin safe for breastfeeding?

Dr. Khanna explains that topical clindamycin is generally considered safe during breastfeeding because systemic absorption is minimal. It should not be applied directly to the breast or nipple area, and patients should consult their healthcare provider before use.

Can topical clindamycin cause fever?

Dr. Khanna notes that fever is extremely rare with topical clindamycin. If fever occurs, it is more likely related to an underlying infection or another cause rather than the topical medication itself, and medical evaluation is recommended.

Why do I feel unwell after using topical clindamycin?

Dr. Khanna explains that topical clindamycin is unlikely to cause systemic symptoms. Local side effects such as redness, dryness, itching, or mild burning are more common. If someone experiences systemic symptoms like nausea or fatigue, they should contact a healthcare provider.

What skin infections will topical clindamycin clear up?

According to Dr. Khanna, topical clindamycin may help treat mild, localized bacterial skin infections caused by susceptible organisms. It is most commonly used for acne rather than deeper or widespread skin infections, which usually require oral antibiotics.

What side effects are common with topical clindamycin?

Dr. Khanna states that the most common side effects include skin dryness, redness, peeling, itching, or mild irritation at the application site. Serious side effects are rare, but patients should stop using the medication and seek medical advice if severe irritation or persistent diarrhea occurs.
