What is Aklief cream and what is it used for?
Aklief (trifarotene) is a topical prescription retinoid used to treat acne. It works by helping the skin renew itself more effectively, which reduces clogged pores and inflammation that lead to breakouts. By targeting specific retinoid receptors in the skin, Aklief helps improve both inflammatory acne (such as red pimples) and non-inflammatory acne (such as blackheads and whiteheads).
Aklief is FDA-approved for the treatment of acne vulgaris in patients 9 years of age and older on the face, chest, shoulders, and back. It is typically applied once daily during the night to affected areas.1

Why is Aklief becoming so popular?

Aklief has become popular because it is the newest prescription retinoid for acne and the first new retinoid molecule approved for acne treatment in more than 20 years. Its active ingredient, trifarotene, is a fourth-generation retinoid that selectively targets the RAR-γ receptor, the most common retinoid receptor in the skin, which helps reduce inflammation and normalize skin cell turnover. This targeted action may help improve acne while maintaining a favorable safety profile. Another reason for its popularity is that it is approved for both facial and truncal acne (such as the chest, shoulders, and back) and can be used in patients 9 years and older, making it useful for many acne patients.2
How to use Aklief correctly?
According to Dr. Anne Allen, a board-certified dermatologist, if you have been prescribed with Aklief, this is how you should use it.

1. Cleanse gently
Clean the affected area with a mild cleanser and pat the skin dry.

2. Wait before applying
This can help reduce irritation when you apply the medication.

3. Apply a thin layer
Aklief is usually used in the evening, unless your dermatologist advises otherwise.

4. Use a tiny amount
Apply a thin layer to the acne-prone areas, not just to individual spots.

5. Avoid sensitive zones
Do not apply Aklief to the eyes, lips, corners of the nose, or broken, sunburned, or irritated skin.

6. Wash hands after use
Wash your hands after applying. This helps avoid getting the medication on sensitive areas by accident.

7. Moisturize
Always use a gentle, non-comedogenic moisturizer to help with dryness or irritation.

8. Use sunscreen daily
Aklief can make your skin more sensitive to the sun, so daily sun protection with SPF 50 is important.

9. Avoid mixing with irritants
Do not layer with other retinoids, harsh exfoliants, or alcohol-based products unless directed.

10. Be consistent
Aklief may show results in as little as two weeks, but it may take several weeks to see major improvement.
Aklief and other medications
Ingredients that can be used alongside Aklief
Niacinamide – Helps calm inflammation and support the skin barrier.
Hyaluronic acid – Provides hydration and helps counteract dryness from retinoids.
Ceramides – Strengthen the skin barrier and reduce irritation.
Gentle moisturizers (non-comedogenic) – Help soothe dryness and peeling.
Broad-spectrum sunscreen (SPF 50) – Essential because retinoids increase sun sensitivity.
Panthenol (vitamin B5) – Supports skin healing and hydration.
Azelaic acid (low to moderate strength) – May be combined carefully to help with acne and redness.
Ingredients to avoid or use cautiously with Aklief
Other retinoids (tretinoin, adapalene, tazarotene) – Using multiple retinoids together can cause excessive irritation.
Benzoyl peroxide – Can increase irritation and dryness when used at the same time.
Strong exfoliating acids (glycolic acid, lactic acid, mandelic acid) – May increase peeling and skin sensitivity.
Salicylic acid (high concentrations) – Can lead to additional dryness and irritation.
Physical scrubs or harsh exfoliants – May damage the skin barrier when combined with retinoids.
Alcohol-based toners or astringents – Can worsen dryness and irritation.
Products containing fragrance or other irritants – May increase skin sensitivity while using Aklief.
Who is Aklief for?
Aklief is intended for patients aged 9 years and older with acne vulgaris, particularly those who have blackheads, whiteheads, and inflammatory pimples on the face, chest, shoulders, or back. It is suitable for adolescents as well as adults who struggle with acne in multiple areas. Dermatologists may recommend Aklief for patients who need a topical retinoid to help unclog pores, reduce inflammation, and prevent new breakouts, especially when acne affects both the face and larger body areas such as the back or chest.3
Who should avoid it/use it with caution?
Aklief should not be used during pregnancy. Like other topical retinoids, it is generally contraindicated for pregnant individuals because retinoids may pose a potential risk to the developing fetus. If you are pregnant, think you may be pregnant, or are planning to become pregnant, you should avoid using Aklief and discuss alternative acne treatments with your healthcare provider. Aklief should also be used with caution in people with very sensitive skin, eczema, or other inflammatory skin conditions, as it can cause irritation such as redness, dryness, or peeling. Because it may increase sensitivity to sunlight, patients using Aklief should apply sunscreen and limit excessive sun exposure. Anyone who has had an allergic reaction to trifarotene or any ingredients in the cream should not use this medication.
